- New Patients: Use the interactive appointment request form below and see below for information regarding new patient appointments.
- Established Patients: Log into the patient portal to request an appointment and update forms. If you have not registered for our secure portal or have not received authorization to begin using the portal, call our office to obtain a registration PIN.
Please do not use this form if you have an urgent medical problem or you need to reschedule an existing appointment. Instead, contact our office at (636) 946-1650. After you submit your request, our appointment scheduler will respond to your request within 48 hours.
New Patients
New patient appointments are longer than follow-up appointments because the doctor will need more time to review information. Since we are a specialty office, we do testing that requires more time than a primary care doctor. Depending on what you are here for, this testing can prolong the appointment.
Another factor that could prolong your appointment is if there is an emergency that requires our attention. We will do our best to make you aware of any situations that could make your appointment longer.
Once you have made your initial appointment, you can download new patient forms to save time in the office. Please be sure to bring them with you to your first visit, as well as any diagnostic films or other test results that may have been ordered or performed by another physician. Completing the forms prior to your visit will help speed up the new patient registration process.
